ACT provides ongoing care to individuals who frequently experience episodes of homelessness, hospitalizations, and/or incarcerations. This ACT Behavioral Health Therapist II provides treatment interventions in a person-centered and patient-driven manner.</span></span></undefined><br><undefined><span style="color: inherit; font-style: normal; text-align: start; text-indent: 0px;"><br style="color: inherit; font-style: normal; text-align: start; text-indent: 0px;"><span style="color: inherit; font-style: normal; text-align: start; text-indent: 0px;" data-pasted="true">Responsibilities include crisis intervention, assessment, treatment planning, care management, individual and group therapy, and coordination of treatment services. Complies with federal, state and agency policies and procedures. Demonstrates the ability to function as a member of a busy, mobile clinical team utilizing advanced clinical skills.
